Creativity Reading

Hello and welcome! Thanks so much for booking a reading. 

Please provide brief answers to the questions below (a few sentences for each should suffice). The information you provide will help me to focus your reading and make the best use of our time together. 

LIVE: If you booked a live reading, please be sure to submit this questionnaire at least 2 days before your scheduled reading. You will recieve a reminder via email to complete the questionnaire.

ASYNCHRONOUS: Once you submit the questionnaire, you will receive your reading via email within two weeks (usually sooner).

All birth chart readings include the following:

• A video recording of your reading (mostly me sharing your chart on the screen and highlighting the planets and aspects as I talk about them)

• A bullet point summary of what was covered in your reading

• A PDF copy of your birth chart

All information will remain strictly confidential.
